Job Description
General Statement of Duties
The Nurse Liaison serves as the clinical bridge between hospital discharge planners, physicians, and our senior living community. This role is responsible for evaluating potential residents for admission, coordinating smooth transitions, and building strong referral relationships to ensure quality care and appropriate placement within our continuum of services.
Essential Job Functions
Assess clinical appropriateness for admission to long-term care, short-term rehab, memory care, or assisted living.
Conduct pre-admission assessments (remote or on-site at hospitals).
Collaborate with hospitals, case managers, and families to coordinate smooth transitions.
Serve as a clinical resource during the admissions process, ensuring accurate handoffs to the care team.
Maintain relationships with referral sources including hospitals, physicians, and other healthcare providers.
Educate referral sources and families on the levels of care and services available.
Track referral data and outcomes for reporting and continuous improvement.
Participate in marketing/outreach events to promote the community’s services.
Support census development and occupancy goals by converting qualified referrals into admissions.
Job Qualifications
Registered Nurse (RN) license in good standing, required.
5 + years clinical experience in acute, post-acute, or long-term care setting, required.
5+ years of experience in case management, admissions, or liaison roles strongly preferred.
Strong leadership, communication, and organizational skills.
Ability to work independently and collaboratively.
Must have a valid driver’s license.
Physical Requirements
Primarily field-based; travel to hospitals, physician offices, and other healthcare settings is required.
May involve evenings or weekend availability for urgent referral needs.
