What You'll Do:

As an Occupational Therapist (OT) in our home health team, you'll:

Conduct comprehensive assessments to evaluate patients' needs and develop individualized treatment plans.
Provide skilled occupational therapy services in patients' homes, focusing on areas such as:
Home safety and accessibility
Medication management
Wheelchair seating and positioning
Pain management
Falls prevention
Management of chronic diseases and dementia
Family and caregiver training
Collaborate with an interdisciplinary care team, including case managers, nurses, and other therapists, to ensure coordinated and holistic patient care.
Serve as a patient advocate and educator, empowering individuals to achieve their functional goals.
Document patient progress and maintain accurate records using electronic charting systems.
Utilize provided communication tools to stay in contact with the rest of the interdisciplinary team.
What You'll Need:

Current and unrestricted California Occupational Therapist License.
Current BLS/CPR Certification.
Valid California Driver's License.
Strong knowledge of local community resources.
Understanding of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Home healthcare experience (highly preferred).
Experience working with high-acuity patients (a plus).
Experience working within an interdisciplinary team (highly desired).
Case management fundamentals (highly desired)
Self-directed, empathetic, and a critical thinker.
Excellent interpersonal, communication, and leadership skills.
Strong organizational skills.
Computer literacy and competency. All candidates will be required to complete an assessment prior to an in-person interview.
Compensation:

Wage range: $120,000 - $130,000 per year, commensurate with relevant experience.
